Dental Insurance

• Company Paid Premium
• Large network including approximately 97% of local providers
• Annual Deductible, $25 per individual, $75 per family
• Pays 80% of Diagnostic, Preventative, Basic, Endodontic and Periodontic services
• Covers many other services

Retirement Plan

Designed to provide protection to employees and their families at retirement
Eligible after working 1000 hours during a 12-month period
Vesting is at 20% each year until employees are 100% vested after 5 years
Company contribution is discretionary and determined by the board of directors
