Deductible:
Single $500 per person, per calendar year
Family $1000 per calendar year not more than $500 per person
Out-of -Pocket:

Coinsurance (amount of covered charges you pay after the deductible):
Generally, 20% Certain services are exempt from coinsurance
Emergency Room Services:
$100 Co-Pay
Illness:
Plan pays 80% after the Calendar Year Deductible In-Network, Out of Network 60%
Physician Services:
Plan pays 80% after the Calendar Year Deductible In-Network, Out of Network 60%
Hospital Services:

Diagnostic X-Ray and Lab:

Inpatient Mental Health:
Plan pays 80% after the Calendar Year Deductible.
Services are limited to 73 days of care per calendar year for mental health and chemical dependency care combined.
Outpatient Mental Health:
Plan pays 80% after the Calendar Year Deductible.
Services are limited to 40 hours of treatment per calendar year for mental health and chemical dependency care combined.
Chiropractic Services:
Plan pays 80% after the Calendar Year Deductible
Home Health Care:
Plan pays 80% after the Calendar Year Deductible.

Dental Insurance
- Company Paid
- Large network including approximately 97% of local providers
- Annual Deductible, $25 per individual, $75 per family
- Pays 80% of Diagnostic, Preventative, Basic, Endodontic and Periodontic services
- Covers many other services
Retirement Plan
Designed to provide protection to employees and their families at retirement
Eligible after working 1000 hours during a 12-month period
Vesting is at 20% each year until employees are 100% vested after 5 years
Company contribution is discretionary and determined by the board of directors. Historically the company has contributed 15% of the employees' eligible wages. However this is not guaranteed.
